The Inner City Family Health Team is committed to delivering comprehensive and integrated primary care to homeless and previously homeless individuals in Toronto, with a specific emphasis on current and former residents of Seaton House. Our approach is focused on addressing their health, housing, and social care needs as they transition to community-based living.

Our team of dedicated healthcare professionals work seamlessly to provide multidisciplinary care, recognizing that the health of our patients is closely intertwined with their housing and social circumstances. We strive to provide holistic care that goes beyond traditional medical services, addressing the unique challenges faced by individuals experiencing homelessness or those who have previously experienced homelessness.

Our services are designed to meet the specific needs of our patients, including medical care, mental health support, addiction services, housing support, and social care interventions. We work collaboratively with other community partners and agencies to ensure that our patients have access to the resources and support they need to achieve optimal health and well-being.

We understand the importance of a community-based approach to care, and we are dedicated to providing compassionate and culturally sensitive services that are tailored to the unique circumstances of our patients. Our goal is to empower individuals who are homeless or previously homeless to transition to community-based living with dignity, respect, and comprehensive care.
